With a view to improve and simplify the differential and tactical approach to the treatment of ulcerative gastroduodenal bleeding, an algorithm for therapeutic and diagnostic procedures has been developed by authors. Clinical and laboratory and endoscopic criteria for a high risk of bleeding have been marked, and a combination of the both of them is an indication for performing urgent surgery. Our experience in the treatment of 493 patients, with gastroduodenal ulcer bleeding, was shown in the Municipal Emergency Hospital in Rostov-on-Don. 79 (16%) patients were operated. Final endoscopic hemostasis was achieved in 414 patients , including 84 (17%) after recurrent bleeding. The obtained postoperative data and overall mortality rate shows the efficiency of this method, with its application as a means of determining the surgical tactics in the treatment of patients with gastroduodenal ulcer bleeding in surgical departments in order to improve the treatment results of this disease.
