PANCREATODUODENECTOMY TO THE EMERGENCY ROOM

The aim of this retrospective study to evaluate the possibility of pancreatoduodenectomy (PDE) in the treatment of patients with adenocarcinoma of the pancreatic head in emergency room. The results of the examination and surgical treatment of 82 patients with tumor formation pancreatoduodenal zone. The survey results revealed: at 64 – Cancer билиопанкреатодуоденальной zone, with 11 – pseudotumor pancreatitis complicated by hyperbilirubinemia, in 7 – Ovarian pancreas head, complicated by obstructive jaundice. Radical surgery, the PDE, performed in 10 patients (8.2 %), and palliative interventions – 72 patients (91.8 %). Death in the early postoperative period (6–7 weeks after surgery) was observed in 2 patients. The cause of death was the failure of pankreatoeyunoanastamoza. Of all operated 8 patients were discharged in satisfactory condition at 16–48 days after surgery. The long-term results have been traced in 6 patients in a period of 2–5 years – were no fatal outcomes. Thus, patients in the emergency room comes late, against which developed complications, most of the patients before admission to the surgical ward 2–4 weeks were treated by a physician or an infectious disease. Late diagnosis results in low percentage of radical surgery. Large traumatic surgery, and tumor expressed holemicheskoy intoxication due to a significant number of complications in the postoperative period and high mortality. PDE still belongs to the category of high risk of life-threatening complications, which limit the range of radical surgery. Requires a comprehensive search for ways to expand the boundaries of rational radical pancreatic head cancer surgery and chronic pancreatitis complicated by jaundice, improved surgical technique and better results.
