TYPES OF RESPONSE OF MICROVASCULAR ENDOTHELIUM IN PATIENTS WITH RESPIRATORY COMORBID CONDITIONS

The functional state of microvascular endothelium in patients with chronic obstructive pulmonary disease (COPD), ischemic heart disease (CHD) and hypertensive disease (HD) has been studied in the course of laser Doppler flowmetry. We have studied the frequency of different types of response of microvascular endothelium to iontophoresis of endothelium-dependent vasodilator in the groups of patients with mononosology (COPD, HD, IHD) and comorbid conditions (COPD+HD, COPD+IHD). In patients with COPD and with IHD we revealed similarities in the response of microvascular endothelium, namely primary or reduced sensitivity of endothelial microvascular receptors when the time of production and action of endogenous vasodilators was reduced, that was observed both at a mononosology, and at a combination of two of these diseases. On the contrary, a characteristic feature in patients with HD is the increased sensitivity of endothelial receptors with the development of microvascular hyperreactivity. The common property of microvascular endothelium, typical of both mononosology (COPD, HD) and comorbid conditions (COPD + HD), was the reduction of the duration of vasodilation, that indicated the development of endothelial dysfunction.
