Comparative analysis of the results of two-staged and one-staged minimally invasive surgical treatment of complicated gallbladder stones disease - choledocholithiasis and stenosis of the papilla of Vater is made in this review. Antegrade papillotomy during laparoscopic cholecystectomy or minimally access cholecystectomy was used as one-stage surgery. Antegrade decompression of the biliary tree allows to decrease the risk of a such heavy complication, as postmanual pancreatitis in comparison with retrograde transpapillary interventions. Usage of the original instrument that we propose facilitates correct orientation of the catgut in the entrance of the papilla of Vater and the technique of antegrade papillotomy when using minimally access during the intervention in the extrabile ducts, that teally releaves the whole time of the operation.
